JOB SUMMARY:
The Administrative Navigator Regional Geriatric Team position assesses real-time bed availability across the region to divert geriatric state hospitalizations to assist with discharge planning from the state hospital. This position also provides support to local nursing homes and assisted living facilities. Assists in the day-to-day business operation of the Geriatric Team and provides administrative support for the team.
ESSENTIAL FUNCTIONS:
Demonstrate knowledge of the services, programs, and populations served by the Geriatric Team.
Develop relationships with nursing home staff and provide support.
Assesses real-time bed availability across the region to divert geriatric state hospitalizations or to facilitate discharge planning from the state hospital. Assist with establishing bed availability across the region through ongoing, frequent contact with nursing homes and assisted living facilities.
Meetings/Trainings – Attend meetings held for a variety of purposes and in-service trainings. Keeps mandatory training up-to-date.
Reimbursement – Obtain individuals’ payer information for the purpose of verifying benefits and coverage. Complete documentation regarding collection attempts.
Reception – Answer telephone, fax information, take and distribute messages for staff. Greet individuals and accompanying caregivers, vendors, guests and other staff directing them to the appropriate location. The opening, date stamping, and distribution of mail. Provide contact information for community resources.
Assist in day-to-day operations which may vary but may include: managing staff’s schedules, manage program vehicles; submit charge account documentation to the administrative office; submit work orders as needed; purchase and order supplies as needed; complete daily schedule reviews; assist in generating reports as needed.
Records Management – Manage individuals’ information in the electronic health record in accordance with Agency and regulatory requirements. Scan and attach information into the individuals’ electronic health record. Process requests for record information within fourteen (14) days of receipt, and after the approval from the Director Regional Geriatric Team.
OTHER DUTIES:
Perform such other duties as may be assigned by supervisor.
QUALIFICATIONS:
Excellent knowledge, skills, and abilities in word processing, spreadsheets, and database systems, including knowledge of electronic healthcare record (e.g., clinical record management and reimbursement billing functions).
Good skill and ability in relating to and coordinating with other health/human service agencies.
Ability to work independently yet function effectively as a team member with multiple supervision and stakeholder guidance (e.g., Regional Management Group, CSBs, or State Facility Leadership, etc.) and with liaisons and emergency teams in the region.
Ability to set priorities and effectively manage time and projects.
Ability to assist individuals with healthcare system issues/barriers.
Ability to communicate effectively with individuals, family members, staff, referral sources, and the general public.
Good oral and written communication skills.
Knowledge of behavioral health confidentiality guidelines.
Job Requirements
EXPERIENCE/EDUCATION REQUIRED:
Bachelor’s degree from an accredited college or university with a degree in a human services-related field or healthcare management is required.
Valid Driver’s license with a safe driving record.
