Role Description The Case Manager is a full-time, hybrid role based in Jakarta, Indonesia, with some flexibility to work from home. The Case Manager will coordinate patient care across multiple services and facilities, ensuring continuity and quality of care from admission through discharge and follow-up. Daily activities include assessing patient needs, developing care plans, organizing referrals, and communicating with healthcare professionals, patients, and families. The individual will monitor treatment progress, help navigate insurance and administrative procedures, and document all case-related information accurately. The role also involves collaborating with multidisciplinary teams, promoting patient education, and supporting the use of digital tools to optimize care pathways and patient outcomes.

Qualifications

  • Demonstrated skills in patient assessment, care planning, and case coordination, with the ability to manage multiple cases simultaneously.
  • Strong communication and interpersonal skills for working with patients, families, and multidisciplinary clinical teams.
  • Proficiency in documentation, data entry, and use of digital health systems or electronic medical records.
  • Knowledge of healthcare processes, insurance or reimbursement procedures, and community or social support resources.
  • Bachelor’s degree in Nursing, Social Work, Psychology, Public Health, or a related field; relevant professional licensure or certification is an advantage.
  • Experience in hospital, rehabilitation, long-term care, or similar clinical settings is highly beneficial.
  • Ability to work effectively in a hybrid environment, demonstrating reliability, time management, and self-organization.
  • Fluency in English and Bahasa Indonesia, with cross-cultural sensitivity and commitment to non-discriminatory, patient-centered care.
