What You’ll Do
- Serve as the primary point of contact for individuals, families, and support teams, facilitating strong relationships and person-centered planning to coordinate services and supports that promote individual goals and needs.
- Manage an assigned caseload while maintaining accurate, timely, and compliant documentation to support effective service coordination, continuity of care, and quality outcomes for individuals.
- Complete assessments, evaluations, and required reports to support person-centered service planning and ensure compliance with state and agency requirements.
- Develop, review, and revise Individual Service Plans using the OhioISP template, along with Individual Budgets, to reflect each individual’s goals, preferences, needs, and changing circumstances.
- Connect individuals to community resources, services, and supports that promote independence, inclusion, and quality of life.
- Advocate for individuals and families while empowering individuals to make informed choices and actively participate in decisions affecting their lives.
- Monitor the delivery, quality, and effectiveness of services and supports to ensure individuals receive the supports outlined in their plans.
- Participate in a rotating on-call schedule to provide 24-hour crisis response, support, and coordination when urgent situations arise.
- Respond promptly to health and safety concerns by coordinating appropriate resources and supports, making referrals, and facilitating interventions as needed to promote individual well-being and safety.
Qualifications
- Bachelor’s Degree (required)
- Eligibility for SSA certification in accordance with OAC 5123:5-02
- Valid Ohio driver’s license and proof of automobile insurance
- Ability to participate in a rotating 24-hour on-call schedule
- Ability to obtain and maintain First Aid and CPR certification

Pay: $23.89 – $35.86 per hour
